2. By submitting this Expression of Interest Form, I am aware that this is not the adoption application

3. I am aware and agree that I own the property listed in this Expression of Interest Application

4. I am aware that there is an adoption fee for this puppy/ dog which is to be advised by No Pup Cleft Behind. And I agree that it is to be paid in full prior to receiving the animal

5. I am aware and agree that any cleft surgery performed (if any) on the above mentioned dog, may or may not hold as the dog grows and develops throughout its life

6. I agree that all information provided by myself, is true and correct and not false or misleading in any way
